For Potential Service Recipients

At Caliber Optima Care, we focus on putting people first. We’re dedicated to helping individuals with intellectual and developmental disabilities live happy and fulfilling lives. Our Sponsored Residential Homes offer a warm, family-like environment where individuals can make their own choices, feel supported, and plan for their future with the help of our caring Providers and their families.

We believe everyone has unique strengths and needs, and we build our care plans around what’s most important to each person. Our goal is to create a safe and supportive space where individuals can grow, learn, and enjoy a high quality of life.

Our homes provide 24-hour care for adults who are transitioning from their family homes or other programs. We carefully assess each person’s needs to make sure they feel comfortable and supported in their new home. Every person we support has an Individual Service Plan (ISP) that is created with them and their support network, focusing on their goals and what matters most to them.

We teach important life skills, like self-advocacy, healthy living, and independence, so each person can reach their full potential. We also work closely with families and guardians, keeping them informed and involved every step of the way.

For Potential Families

At Caliber Optima Care, we take great pride in the programs we offer to communities across Virginia. We are just as proud of the families we select to become Sponsored Families, providing care to individuals with intellectual and developmental disabilities. Becoming a Sponsored Family involves several steps, and we’re here to guide you every step of the way.

One of the first steps is completing a Home Study. During this process, a member of our team will visit your home to assess whether it’s a good fit for supporting an individual with a disability. We’ll also make sure your home meets all necessary requirements and that you have the proper documentation.

We’re committed to helping our Sponsored Families succeed. Here’s how we support you:

  • We guide you through the pre-placement and application process, checking in regularly to ensure everything is going smoothly.

  • We help you find and complete the necessary training, both during orientation and annually.

  • We assist you in preparing for and welcoming the individual into your home.

  • For the first 60 days after someone moves in, we’ll check in with you at least once a week and be available whenever you need us.

  • After the first 60 days, we’ll continue to check in at least once a month and remain available for any support you need.

  • We’ll review your service delivery and documentation to make sure you’re comfortable and to provide any additional support you might need.
